Not all patients with kidney disease will require dialysis. Early detection and treatment is essential to preventing dialysis.

 Please bring your insurance cards, a photo identification card, referral (if required by your insurance) and all medications you currently take.

We have a few forms that will need to be complete prior to meeting the doctor. Forms are simple and can be complete upon your arrival to the clinic, but if you prefer to fill it ahead of time please talk to one of our clinic team member about how to obtain it.

Co-payments or any other payments not covered by your insurance, including deductibles or out of pocket expenses, need to be paid at the time of service. 

We do accept checks. We also accept Visa, Discover, Master and American Express cards. Also we accept self-pay.

Our clinic participates with most of the major payers and with a variety of HMO’s. Please talk to one of our clinic team member about your specific plan.

You can expect that your doctor will review with you your medical history, and your current medications. You will also receive a physical exam, and engage in a discussion about your care.
